Prior to your appointment, we will send you a New Patient form link to complete online via email or sms (you may also opt to complete a paper copy on arrival if preferred) for our chiropractor to review. During your appointment, the chiropractor will conduct a thorough history & examination to see what problems are present and will treat accordingly.
